Hormone Laboratory Testing Options

Patients may choose:

• $50 per month – Discounted hormone lab program
• Pay per lab draw – Typically more expensive
• Lab draws billed to insurance – insurance may cover little to no expense related to hormone and other lab testing; Evolution Wellness is in no way responsible for lab service expenses. Any cost not covered by insurance is the patient's responsibility.

• Local pharmacy (insurance may be used if applicable. At Evolution Wellness, we use hormones to optimize wellness and vitality. Insurance will not cover medications for these indications, therefore; we will not complete prior authorization requests for medication coverage related to hormone optimization).
