You do not need to transfer your primary care to us.

We accept most major commercial insurance plans, including WellCare.

Please come fasting to your new patient visit.

Before Your Appointment

Please bring

  • A valid photo ID
  • Your current insurance card (if applicable)
  • A payment method for any copays, deductibles, balances, or self-pay services
  • An updated medication and supplement list if there have been any changes
  • Any outside medical records, laboratory results, or imaging you would like your provider to review

Complete your forms

Please complete all questionnaires and paperwork through the OptiMantra Patient Portal before arriving whenever possible.

Arriving for your appointment

Please arrive 15 minutes early if your insurance, pharmacy, address, or demographic information has changed, or 5–10 minutes early for routine appointments.

Appointment Types

Annual Wellness Visits

Comprehensive preventive care visits including physical examinations, age-appropriate screenings, routine laboratory review, and personalized health recommendations.

Follow-Up Visits

Ongoing management of chronic conditions, medication management, thyroid disorders, diabetes, high blood pressure, high cholesterol, hormone optimization, weight management, and lab review.

Sick Visits

Evaluation and treatment of acute illnesses and injuries including respiratory illnesses, ear infections, sore throat, UTIs, skin conditions, minor injuries, and other urgent medical concerns.

Hormone Optimization Visits

Personalized evaluation and treatment for perimenopause, menopause, low testosterone, PCOS, thyroid dysfunction, fatigue, low libido, sleep disturbances, and metabolic concerns.

Weight Management Visits

Medical weight management including GLP-1 medication follow-up, nutrition counseling, lifestyle coaching, medication adjustments, progress reviews, and InBody body composition analysis.

Laboratory Visits

Available for annual wellness labs, hormone monitoring, chronic disease management, specialty laboratory collections, and follow-up testing. Some tests require fasting — unless otherwise instructed, avoid food for 8–12 hours prior.

Telehealth Visits

Many follow-up appointments for established primary care patients can be completed via secure video visit when medically appropriate. You'll receive a secure text message with your video link after nursing check-in.

IV Therapy & Wellness Injections

Customized IV therapies for hydration, immune support, athletic recovery, energy, beauty, longevity, hangover relief, and cognitive support. Wellness injections include B12, MICC/Lipo-B, Vitamin D, and other provider-recommended therapies. Walk-ins welcome.

InBody Body Composition Scan

Detailed analysis of body fat percentage, skeletal muscle mass, visceral fat, hydration status, basal metabolic rate, and weight trends. Wear light clothing and stay well hydrated for best results.

BioScan Visits

BioScan appointments use bioenergetic technology to evaluate wellness patterns and guide individualized lifestyle and nutritional recommendations. BioScans complement — not replace — medical evaluation.

Patient Policies

Cancellation Policy

Please provide at least 24 hours' notice if you need to cancel or reschedule. Missed appointments or late cancellations may be subject to a cancellation fee.

Prescription Refills

Contact your pharmacy first. Please allow at least 48 business hours for routine refill requests. Controlled medications require regular follow-up appointments and cannot be guaranteed after hours or on weekends. Prescription requests are not processed through social media.

Lab Results

Lab results are reviewed with your provider at a follow-up visit or via telehealth. We walk you through what your numbers mean and what to do next.

Insurance & Billing

If your insurance changes, please notify our office before your appointment. Many wellness services are not covered by insurance, including hormone optimization memberships, specialty lab testing, IV therapy, wellness injections, BioScan services, and aesthetic treatments. Financial responsibility for non-covered services will always be discussed before treatment.

Medical Emergencies

If you experience chest pain, difficulty breathing, stroke symptoms, severe allergic reactions, uncontrolled bleeding, or any life-threatening emergency — call 911 or proceed to the nearest emergency department immediately.
